Park Forest-based vendors
Businesses based in Park Forest—whether commercial or home-based—must register with the Village to participate in community events. This requirement applies to all businesses, regardless of size or type.
Registration ensures compliance with local ordinances. In addition, any business that prepares, handles, or sells food must comply with all applicable health and safety regulations.
All businesses must be properly registered with the State of Illinois and in "good standing," whether operating as a Corporation, LLC, or under an Assumed Business Name (DBA).

Food Vendor

Location of Event(s) - The Village must be listed as an additional insured. Therefore, the "certificate holder" must say:

- You are required to provide a ANSI Accredited Food Protection Manager Certificate
- You must also provide a Illinois Retail Sales Tax Certificate that can be obtained by clicking here

- Please note that there is a flat fee of $55 for each event that covers the health inspection

How to Submit Applications
- Applications must be submitted to Arika Rogers, Business Licensing, at arogers@vopf.com
- INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED
Important Notes
- First-come, first-served basis
- Please allow up to five business days for processing if your application is complete
- Peddlers will receive their certificates from the event coordinator on the day of the event
- Food vendors will be inspected by the Cook County Department of Public Health at the beginning of each event
- Your event certificate will be issued on-site once you pass the inspection
